REQUIREMENTS

1. Duly Accomplished Admissions Form
2. 1pc. recent 1" x 1" ID photo (Student)
3. 1pc. recent 1" x 1" ID photo colored (each Parent)
4. Photocopy of the Church Marriage Contract of the Parents
5. Photocopy of the Birth Certicifate of the Student Applicant
6. Photocopy of the Baptismal Certicifate of the Student Applicant
7. Photocopy of Report Card (of most recently completed school year or quarter of the current school year)
8. Sealed Recommendation Forms (for Grade 2 and beyond)
9. PAREF Statement of Principles / Signed Parents' Undertaking
10. Additional Requirements for Non-Filipino / Foreigner
    • Transcript of records from the forreign school
    • Photocopy of the Alien Certification of Registration (ACR) or Immigration Certificate of Registration(IRC)
    • Photocopy of Passport
    • Original Notarial Certificate (for Koreans)
